Carlos Rivera leads the operational engine of Alianza Health Partners as Chief Operating Officer. He is responsible for the day-to-day performance of Alianza’s clinical network, MSO services, provider staffing operations, and the development and launch of new Community Care Centers across Puerto Rico’s municipalities.

Carlos brings a builder’s mindset to healthcare operations. Throughout his career he has opened new clinical sites, restructured underperforming practices, and built revenue cycle functions from scratch in resource-constrained environments. He understands that operational excellence in healthcare is not about efficiency for its own sake—it is about creating the conditions in which clinicians and care teams can do their best work for patients.

At Alianza, Carlos oversees a cross-functional operations team responsible for credentialing, payer contracting and enrollment, facilities management, supply chain, and human resources. He also leads the operational integration of newly affiliated practices into the Alianza MSO, managing the onboarding process that transforms independent clinics into high-performing participants in Alianza’s value-based care network.

Carlos is known for his direct communication style, his commitment to staff development, and his ability to identify and eliminate the operational bottlenecks that drive physician frustration and patient dissatisfaction. He is fluent in English and Spanish and holds deep personal and professional roots in the Puerto Rico healthcare community.
