Generic population health tools designed for mainland US markets frequently miss the mark in Puerto Rico—failing to account for language preferences, island-specific infrastructure challenges, or the unique payer mix of the island’s healthcare system. MyChron was conceived and built by Alianza Health Partners to close these gaps. Every feature of the platform reflects direct input from Puerto Rico clinicians, patients, and payer partners.
MyChron operates as the connective tissue of the entire Alianza clinical ecosystem. It aggregates data from EHRs, claims feeds, pharmacy systems, remote monitoring devices, and patient-reported outcomes into a unified longitudinal health record for every attributed patient. Care teams see what they need, when they need it, without navigating multiple disconnected systems.
Population Health Management
MyChron’s population health module stratifies attributed patient panels by risk level, care gap status, and chronic condition burden. Care managers receive automated daily work lists identifying patients who need outreach—those overdue for screenings, with uncontrolled A1C or blood pressure, or recently discharged from the hospital. Predictive risk models, trained on Puerto Rico-specific claims and clinical data, identify patients at highest risk for near-term hospitalization so interventions can be deployed proactively.
Remote Patient Monitoring
Alianza deploys FDA-cleared remote monitoring devices for patients with diabetes, hypertension, heart failure, and COPD. Blood glucose meters, blood pressure cuffs, pulse oximeters, and connected scales transmit readings directly into MyChron, where out-of-range values trigger automated alerts to the care team. Clinicians can review trends, adjust medications via telehealth, and document interventions—all within the same platform. Remote monitoring has been shown to reduce hospital readmissions and improve chronic disease control, and Alianza’s deployment model is designed to be operationally sustainable at scale.
Patient Engagement and Telehealth
MyChron’s patient-facing mobile application provides bilingual (English/Spanish) access to appointment scheduling, secure messaging with care teams, medication reminders, and health education content tailored to each patient’s conditions. Integrated video visit capabilities allow patients to see their PCP or specialist without leaving home. Push notifications, SMS reminders, and automated call-backs ensure that even patients with limited smartphone literacy can stay connected to their care team. Alianza’s digital health program is designed to meet patients where they are—not to require them to adapt to technology.
