Puerto Rico’s municipal governments are uniquely positioned to address healthcare access disparities at the community level—they know their populations, own facilities, and have the political will to act. What many lack is the clinical infrastructure, operational expertise, and value-based care capabilities needed to run effective health programs. Alianza Health Partners fills that gap through a structured public-private partnership model that leverages municipal assets and political relationships while deploying Alianza’s full clinical and operational infrastructure.

Each Community Care Center is co-developed with the host municipality, designed to reflect local needs, and staffed by clinicians who live and work in the communities they serve. These are not pop-up clinics or temporary programs—they are permanent health infrastructure investments anchored in long-term partnership agreements.

Comprehensive Services Under One Roof

Alianza Community Care Centers are designed as one-stop health destinations. Services typically include adult and pediatric primary care, behavioral health and substance use disorder treatment, women’s health and prenatal care, dental and vision services, pharmacy, laboratory, and radiology. Social services navigation—connecting residents to food assistance, housing support, transportation, and benefits enrollment—is integrated into every center’s care model.

The centers serve as hubs for Alianza’s MyChron population health platform, enabling care teams to identify and outreach to high-risk residents, track care gap closure across the municipal population, and report outcomes to both municipal governments and payer partners.

Value-Based Funding and Sustainability

Unlike traditional municipally funded clinics that depend on annual budget allocations, Alianza Community Care Centers are designed to be financially sustainable through a combination of Medicaid managed care revenue, Medicare Advantage capitation, FQHC look-alike funding, and municipal co-investment. Alianza’s value-based contracting expertise ensures that the centers generate quality bonuses and shared savings that flow back into center operations and municipal health programs.

Disaster Resilience and Emergency Preparedness

Puerto Rico’s experience with Hurricane Maria underscored the critical importance of distributed, community-based healthcare infrastructure that can function when hospital systems are overwhelmed or inaccessible. Alianza Community Care Centers are built with generator backup, emergency supply caches, and telecommunications redundancy so they can serve as healthcare anchors during natural disasters and public health emergencies. Our partnership with municipal emergency management offices ensures each center plays a defined role in community disaster response plans.
