Puerto Rico has historically suffered from specialist shortages and fragmented referral pathways that force patients to travel to the mainland or forgo care altogether. Alianza Health Partners addresses this gap by building a coordinated specialty care network anchored in our primary care infrastructure. Every specialist in the Alianza network operates under shared clinical protocols, uses integrated care records, and participates in joint care planning with the referring primary care physician.

Our specialty care coordination platform tracks every referral from the moment it is placed to the moment the consultation note is received by the PCP. No referrals fall through the cracks. Patients receive appointment reminders, transportation assistance if needed, and follow-up calls post-visit to ensure recommendations are implemented.

Network Breadth and Depth

The Alianza specialty network spans cardiology, endocrinology, pulmonology, nephrology, orthopedics, behavioral health, and obstetrics/gynecology. We credential and contract with both independent specialists and hospital-employed physician groups, giving patients access to the right expertise regardless of affiliation. Specialists are measured on access metrics—appointment wait times, consultation turnaround, and patient satisfaction—with network participation contingent on meeting Alianza quality standards.

Integrated Care Planning

Joint care conferences between PCPs and specialists are a cornerstone of the Alianza model. For patients with complex or multiple chronic conditions, our care management team facilitates structured multidisciplinary reviews that align treatment goals, eliminate duplicative testing, and reconcile medication regimens. This integrated approach reduces avoidable hospital admissions and shortens lengths of stay when hospitalization is necessary.

Telemedicine Specialty Access

For communities in rural municipalities and underserved urban areas, Alianza deploys telemedicine-enabled specialty consultations. Our Digital Health platform (MyChron) supports secure video visits, remote monitoring data sharing, and asynchronous specialist review of diagnostic results. Patients who previously waited months for a specialist appointment can now receive timely expert guidance from the comfort of their local community care center or their own home.
