San Juan, PR – December 5, 2025 – Alianza Health Partners today announced the execution of its first partnership agreements with Puerto Rico municipalities for the co-development and operation of Alianza Community Care Centers. The agreements mark a significant milestone in Alianza’s mission to extend comprehensive, value-based primary care to every corner of the island, including the rural and underserved communities that have historically lacked consistent access to quality healthcare.

A New Model for Municipal Health Infrastructure

The Alianza Community Care Center model pairs the municipal government’s unique assets—local relationships, existing facilities, and political will to improve community health—with Alianza’s clinical infrastructure, operational expertise, and value-based payer contracting capabilities. Under the partnership agreements, Alianza will design, staff, and operate the centers, while municipalities contribute facilities, community outreach support, and engagement with local social service networks.

Comprehensive Services in Every Center

Each Alianza Community Care Center is designed to deliver a comprehensive suite of services under one roof:

  • Primary and Preventive Care: Adult and pediatric primary care, comprehensive wellness visits, and preventive screenings aligned with USPSTF recommendations
  • Chronic Disease Management: Structured disease management programs for diabetes, hypertension, asthma, COPD, and cardiovascular disease
  • Behavioral Health Integration: On-site behavioral health counseling and substance use disorder treatment co-located with primary care
  • Women’s Health: Prenatal care, gynecology, and maternal health services
  • Social Services Navigation: Food insecurity screening, housing assistance referrals, benefits enrollment support, and transportation coordination
  • Pharmacy and Laboratory Services: On-site dispensing and basic laboratory testing to reduce barriers to medication access and diagnostic follow-up

All services are delivered bilingually in English and Spanish, with community health workers drawn from the local community serving as cultural and navigational bridges.

MyChron Integration and Population Health

Each Community Care Center operates on the MyChron population health platform, enabling Alianza and municipal partners to track health outcomes across the entire municipal population, not just those who walk through the center’s doors. Proactive outreach programs identify unengaged high-risk residents and bring them into care before health crises occur.

Municipalities receive regular population health dashboards showing care gap closure rates, disease control metrics, and preventive service utilization—transparent evidence of the investment’s impact on community health.

Disaster Resilience

In recognition of Puerto Rico’s vulnerability to hurricanes and natural disasters, each Community Care Center is designed with generator backup, emergency supply caches, and telecommunications redundancy. Every center will have a defined role in its municipality’s emergency response plan, ensuring that healthcare access continues when it is most needed.

Looking Ahead

Alianza expects to open its first Community Care Centers in 2026, with a goal of establishing a presence in municipalities across all regions of Puerto Rico over the following three years.
