Job Description

Join Alianza's primary care network as a clinical leader delivering value-based, patient-centered care to a diverse Puerto Rico population.

Responsibilities

  • Manage a panel of 1,200-1,500 attributed patients under value-based care contracts
  • Conduct comprehensive wellness visits, chronic disease management, and acute care encounters
  • Collaborate with care managers, behavioral health specialists, and social workers on complex patient cases
  • Participate in quality improvement initiatives and HEDIS measure performance
  • Engage with the MyChron population health platform for panel management and care gap closure
  • Mentor and supervise advanced practice providers as panel warrants

Requirements

  • M.D. or D.O. from accredited medical school
  • Board certified or board eligible in Internal Medicine or Family Medicine (ABIM or ABFM)
  • Active Puerto Rico medical license or ability to obtain within 90 days
  • DEA registration and ability to obtain Puerto Rico controlled substance registration
  • Bilingual English/Spanish strongly preferred
  • Commitment to value-based care and population health principles
